Responsibilities

• Administer medications and treatments

• Monitor patient condition and vital signs

• Provide wound, tracheostomy, and daily nursing care as needed

• Assist with feeding tubes and personal care support

• Document changes and communicate with care team

• Promote client comfort, safety, and independence

Qualifications

• Active RN or LPN license required

• Strong communication and clinical assessment skills

• Home care experience preferred but not required

• Compassionate, dependable, and professional
